Does it handle commas inside quoted fields?

Yes - parsing uses a real CSV reader, not a naive split on the delimiter, so a quoted field containing the delimiter itself still parses correctly.

What's the JSON output shaped like?

An array of objects, one per row, each keyed by the header row's column names.

What if a Markdown cell contains a pipe character?

It's escaped automatically so an embedded | can't break the table layout.
